I'd recommend doing your send to your external fx through the iCA4; Presuming your instrument is in input one - use iConfig to route that input to both an iPad input as well as one of the iCA4 outs. From a quick look at the Duo, the first input, labeled instrument, is switchable to hi z which should be compatible with your presumably 1/4" connected line6 box. This also presumes the output of the line 6 is designed to plug straight to a guitar amp....

Might depend on how the midi file was recorded - for instance I've downloaded/imported midi midi clips that have them embedded.
Perhaps an erroneous press on your keyboard?

It kinda sounds like you may have a program change embedded in your midi track.
Double tap on your midi track, and on the "notes" drop down, see I'd Program Change is in bold text. If so, select it, and either delete any PC messages or set them to the preset you want.....

This possibly could be tamed using multiband compression - a narrow frequency range could be defined around the pop, and just that area would be compressed, take some of the energy out of it.
